What are fillers?

Most dermal fillers are made from Hyaluronic Acid. Hyaluronic Acid is a naturally occurring substance in your skin that helps to hydrate and add volume. We only use Hyaluronic Acid fillers – such as Juvéderm because there is a medication that dissolves Hyaluronic Acid fillers. Therefore, in the rare instances that a patient has lumpiness, asymmetry, or any other outcome that they are not totally happy with, the filler can be dissolved.

What do fillers do?

Fillers can be used to lift wrinkles, furrows and depressions around the nose and mouth. Fillers can also be used to plump the lips and decrease those smokers’ lines that you can get even without smoking. It can reduce the appearance of under-eye bags, and lift the cheekbone, making you look younger and refreshed instantly.

How do fillers differ from Botox?

Fillers act by “filling in” wrinkles or creases in your face. It works best in the lower face where lines tend to be caused by loss of volume under the surface of your skin – especially the lines around your mouth and nose. In contrast, Botox works by relaxing muscles in your face that pull your skin into creases or furrows. It works best on the upper face where the muscles of your forehead cause lines between your brows and across your forehead. For best results, both are used together.

How soon will I see results from my dermal filler treatment?

Results are immediate and natural. Most patients need one treatment to achieve optimal wrinkle smoothing, and the results last up between 9 months and 18 months depending on what filler you use (Generally the more “cross-linked” a filler is, the longer it lasts.

Patients will typically be slightly red in the injected areas for 24-48 hours. About 1/3 of patients will have a small bruise at the injection site as well. Filler can typically be felt under the skin for a couple of weeks after the injections are performed.

Are there any potential side effects?

Side effects of Filler injections may include slight bruising, redness or swelling. Most side effects are mild and last only a few days. The most common side effects include temporary injections site reactions such as redness, pain/tenderness, swelling, lumps/bumps, and bruising. As with any injection there is a small risk of infection.

What should be done before a Filler treatment?

It is recommended to arrive for your dermal filler appointment with a clean, cleansed face without make up. Avoid taking aspirin or ibuprofen before your appointment – as these are both blood thinners which can increase bleeding or bruising at the injection site.

What should I do immediately after Filler treatments?

Refrain from vigorous exercise until the following day. Other than that, you can return to your normal activities. Avoid pressure to the injected areas for about two weeks. Fillers remain mouldable for 1-2 weeks, and pressure can deform your filler. The most common culprits? The face cradle on massage tables, tight ski goggles and swim goggles. Normal activities like washing your face or putting on make-up do not have enough force to mold your filler.

Are Filler injections painful?

Injections may cause some discomfort during and after the injection. Most Fillers have Lidocaine (a painkiller) mixed in with the gel which reduces the discomfort soon after it is injected. You may choose to numb the treatment area with topical numbing cream or ice to reduce discomfort – especially around the lips which are very sensitive. You will be a little sore the day you get your injections, but that resolves in 24 hours or so.

How long does treatment usually take?

The injection process takes only about 15 to 30 minutes. If you choose to do topical numbing, we recommend allotting an additional 30 to 60 minutes for the numbing medications to take effect.

Will I be able to feel bumps under my skin?

You will be able to feel the filler under your skin for a couple of weeks, and these bumps can sometimes be seen as well. However, they are not permanent.

As with the springs inside a mattress – the thicker and harder the support system is – the more you can feel the springs when you lie down on it. This is also true of fillers. When fillers are placed under the skin – especially if it is placed in an area where there skin is very thin, or if it is placed very close to the surface, you will be able to feel it when you press on the area with your fingers. If it is placed well – it should feel like a smooth, rubbery “pad” under the skin – giving support to the skin that was beginning to wrinkle.

These bumps will even out over time as the filler dissolves. Fillers remain mouldable for a out a week after they are placed – so if there is a bump that does not go away on its own, it can be “massaged” out with fingers or a Q-tip in a 15-minutes follow-up appointment.

The only fillers that you notice are those that are overdone or misplaced – trust me when I say that you have seen a lot more people with filler than you think you have! Many of the people you think overdid a filler have surgical implants. Getting an ‘overblown’ look with fillers is difficult because of the sheer amount of material it takes to get that look.

I heard there are permanent fillers. Why isn’t everyone using them?

The most important thing to understand with fillers of any kind is that despite having injections, your face will continue to age. Aging is a dynamic process that involves a gradual loss of bone and soft tissue across several areas of the face. As the tissue that supports our skin becomes thinner, and slides down our faces under the influence of gravity, fillers placed in the cheeks and lips that looks good when we are 50 will not look appropriate when we are 60. Permanent filler in the face leads to a face with unnatural proportions that are too big and in the wrong position as the aging process continues. One of the biggest benefits of the Hyaluronic Acid fillers like Juvéderm is that it can be continually adjusted to meet the changing needs of your face.

Can every line and wrinkle be treated with fillers?

There is still a misconception that fillers and Botox are similar. Botox works to relax the facial muscles that cause lines and wrinkles, giving a more relaxed and smoother look to skin. Photorejuvenation is a technical term for the removal of sun damage to the face and body with special laser light treatment. The freckles that turn into patches, the patches that turn into dark spots, and the dark spots that turn rough, are all evidence of sun damage to the skin.

The extra pigment that is stored in the skin cell because of damage to the skin can be targeted with laser technology. The pigment absorbs the heat from the laser light and causes the skin to release that area of damage. How does laser spider vein removal work?

This laser has a wavelength that targets hemoglobin or blood. Each vessel is traced with a small spot of laser light travels through the skin and is absorbed by the blood within the vein based on its colour. As the energy is absorbed by the blood in the vessel, heat is created, and the vessel wall is destroyed. The resulting heat coagulates or clots the blood and destroys the function of the vein. Over time, the vein will be absorbed by the body and will disappear.

How many treatments will I need?

While many clients may need only one treatment to accomplish their goal, others could expect 2 or more treatments for clearing. Treatments are usually 6 weeks to 6 months apart.

How long will it take for my veins to disappear?

This range from days to weeks and possibly months (for the legs). How quickly veins will be reabsorbed will vary for each client’s; location, size and the colour will also influence the healing time.

Will my treated veins come back?

Once a vein is completely dead and reabsorbed, it should be gone. However, a person who has already made spider veins may have the tendency to make more in the future. Any newly developing vein can be quickly treated.

Precautions before treatment?

Do not drink alcohol or smoke for 2 day before and after your treatment.

Do not take aspirin, Ibuprofen or other arthritis medicine for 2 days before and after you treatment. These medications can increase bruising.
You may take Tylenol (acetaminophen) for discomfort.
Do not take ASA or related compounds (aspirin, naproxen, or Ibuprofen) since they may increase bleeding.
Wear loose clothing on the day of your treatment.
Expectations following treatment and what will I see after treatment?
Immediately following treatment, the skin over the treated vessel will appear red and swollen for a few hours after the treatment. The area will be a little red and may show a slight erythema with some swelling.

What are the possible side effects?

While side effects are uncommon, Bruising or darkening of the treated veins may occur. There is a small risk also of a water blister, change in skin pigmentation or scarring. This rarely occurs.

What should I do after a treatment?

After your treatment you may continue your normal daily activities. The application of ice packs is recommended to minimize bruising. Apply ice packs and elevate the legs. 20-30 minute walk is recommended. Avoid sun exposure for one week after treatment. A broad-spectrum (UVA/UVB) sunscreen of SPF 30 or higher should be applied whenever exposed to the sun. If swelling and redness develop after few days, you can take Ibuprofen 400 mg 4 x daily and call us at any time for advice.

Avoid strenuous exercise for 1 week after treatment. Walking is encouraged. No weightlifting and aerobics. No alcohol. Avoid anti-inflammatories for 48 hours, unless taking ASA for heart condition.
